Home Care: From Hospital to Home
We believe that returning home should be a relief, and have created a service to help make the transition as stress-free as possible. To help you rest and continue to heal, our support worker will help with the following:
On the day of discharge:
- Help pack up belongings at hospital and take you home
- Air the house, unpack and put fresh linen on the bed
- Shop for essentials and clear out the fridge
- Prepare and serve a light meal

The following strategies may include in the care plan:
- Strengthening Circles of Care: coordinate family doctor, care manager, care coordinator, and support workers plus allied health or specialists (if required) to make care more efficient.
- Accessible Transportation: visits to family doctor and any follow-up appointments are important to a successful recovery.
- Medication Reminders and Prescription Pickup: pick medications up at the pharmacy and help make sure that Consumers are taking the medications they need, when they need them.
- Nutrition Planning, Food Shopping and Meal Preparation: eating nutritious meals is very important to maintaining good health. It becomes even more important when special dietary requirements are necessary, such as with diabetes or congestive heart failure. We can help ensure compliance with any post-discharge nutrition orders by preparing meals to meet Consumer's specific needs.
